Pathway assessments

Young people leaving care are entitled to an assessment of their needs which takes account of their views and results in an action plan for their future.  This assessment or “Pathway plan” is usually initiated and coordinated by their allocated Social Worker.  This plan outlines their needs and the arrangements for their support as defined in the regulations and guidance on services for young people ceasing to be looked after by local authorities. 

Pathway plans

A pathways plan is developed from the Pathway assessment.  This plan identifies the types of support required to prepare the young person for each stage in their progression.  Their plan is reviewed with the young person every four or six months depending on individual circumstances.

Pathway coordinators

The Pathway coordinator is someone who is identified that will oversee the completion and reviewing of the Pathway Plan.

This person is generally someone who has a good relationship with the young person.

Some young people do not want to have support once they leave care, and this is respected, however, they will remain eligible for the service.  The team operates an “open door policy” and young people can contact our duty service any weekday afternoon for assistance.
