Services are provided based on each individual's own circumstances and needs.
The assessment will be conducted by a member of staff to discuss your needs and those of your carer, including your preferences for your future care.
Your assessment identifies the particular services that are required to meet your needs.
For many people, it will only take a matter of days to complete the assessment and start providing services. For others, the assessment process may take longer.
We will ask for your consent to share the information from your assessment with other health or social care services. In order to allow us to understand your daily living needs staff will ask you who else we should speak to, for example your doctor or your family.
You will be kept informed of what is happening during the assessment process and while your assessment is taking place you may be offered some services to deal with any immediate needs you have.
